Information Practices
Renfrew County and District Health Unit (the Health Unit) is committed to respecting privacy and protecting personal health information in accordance with the Personal Health Information Protection Act, 2004. Health Unit staff collect personal health information directly from individuals or from the person acting on their behalf. Occasionally, personal health information is collected from other sources if consent to do so has been given or if the law permits.
The Health Unit collects, uses and discloses personal health information to:
- provide health services
- receive funding/payment for provision of health services
- plan, administer, and manage internal operations, programs and services
- conduct risk management and quality improvement activities
- teach
- compile statistics, conduct research
- comply with legal and regulatory requirements, and
- fulfil other purposes permitted or required by law
The Health Unit takes steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal. Personal health information is kept confidential and available only to authorized employees.
Access
Individuals may obtain access to or request correction of their personal health record held by the Health Unit, or withdraw consent for some of the above uses and disclosures subject to legal exceptions.
